Obtaining AAC Devices - Medicaid Step 1. A physician writes a prescription for an AAC assessment. Step 2. The facilitator (the person who drives the process, e.g., consumer, family member, case manager) completes the Essential Information Form. Step 3. The facilitator identifies a service provider to perform the assessment. Regarding the evaluation, the facilitator must: a. Schedule the evaluation with the service provider b. Inform the appropriate parties (family members, care staff, etc.) of the appointment c. Arrange for transportation, if necessary d. Provide the Essential Information Form and the physician's prescription to the service provider. Step 4. The service provider performs the evaluation and writes a report. The service provider must: a. Obtain signatures on the Assignment of Benefits Form b. Perform a full evaluation c. Determine the most appropriate device based on needs and costs d. Provide the facilitator and the consumer with a summary of the recommendations e. Complete an evaluation report f. Forward the report to the facilitator, consumer, vendor, Medicaid/MCO, and physician g. Send the information to the physician for use in the Letter of Medical Necessity h. Request a device prescription from the physician i. Inform the beneficiary/family of the information needed for the claim. Step 5. The facilitator provides the Essential Information Form to the vendor. Step 6. The physician writes the equipment prescription and the Letter of Medical Necessity. S/he sends the letter and the prescription to the vendor, facilitator, and Medicaid/MCO. Step 7. Once the vendor receives the Essential Information Form, the prescription, and the Letter of Medical Necessity, s/he prepares the paperwork and sends the Certificate of Medical Necessity to the physician. Step 8. When the physician receives the Certificate of Medical Necessity, s/he completes it and sends it back to the vendor. Step 9. The vendor can now order the equipment. S/he must make sure to: a. Call Medicaid/MCO for authorization b. Submit documentation to Medicaid/MCO c. Order the equipment d. Contact the facilitator, service provider, and the consumer to arrange for delivery of the equipment. Step 10. The service provider performs the equipment setup. S/he must: a. Set up the communication device b. Provide information about the care and operation of the equipment.
