Repairing AAC Devices - Medicaid Do not return the device to the manufacturer until you have completed the following steps! Call the manufacturer. Make sure to: a. Identify the make, model, and serial number of the device b. Have the device with you c. Call the manufacturer's technical support or customer service number d. Work with the manufacturer to determine if the device needs to be returned. If the device needs to be sent away for repairs... Step 1. While on the phone with the manufacturer, be sure to get the following information: a. Return Authorization Number b. Instructions for returning the device. Step 2. Collect the following documentation to send to the manufacturer along with the device: a. Copy of the Essential Information Form b. Copy of the AAC Request for Repair Form (keep a copy for your records). Step 3. Coverage of the repair is dependent on the following items being fulfilled: a. The Certificate of Medical Necessity was sent to the vendor by the physician b. There is documentation that the current device is still appropriate c. The manufacturer is enrolled as a DE provider or repair can be facilitated through a DME vendor who is enrolled. Step 4. Is the cost of the repair under $300? If yes, skip to Step 6. If no, continue on to Step 5. Step 5. If the cost to repair the device is over $300, Medicaid will require the following: a. Authorization b. Certificate of Medical Necessity (CMN) from the physician c. A letter from the Speech-Language Pathologist indicating that the device needs repaired and that the user continues to use the device as a primary means of communication. Step 6. Send the device to the manufacturer for repair. Be sure to: a. Send the device as instructed by the manufacturer b. Include the Request for Repair Form and the documentation listed above.
